2024 CORPORATE/ORGANIZATION ANGEL TREE ADOPTION REQUEST FORM

Any corporation, church, civic organization, or family looking to adopt a group of 10 angels or more from our Angel Tree, please fill out the form below.

Please note, that angels are allocated on a first come first served basis. The final date to submit a request for identified angels is October 25, 2024. After this date, we will evaluate availability after we have fulfilled current requests. We will reach out to you to confirm your request or discuss alternate options at that time.
